1. ACCURACY AND COMPLETENESS OF INFORMATION
You certify that all information, statements, and documents provided by you in connection with this application—including but not limited to your resume, curriculum vitae, cover letter, and interview responses—are true, accurate, and complete to the best of your knowledge. You understand and agree that any misrepresentation, falsification, or material omission of fact may result in the immediate disqualification of your application, the revocation of any conditional offer of employment, or the immediate termination of your employment if discovered after hire, regardless of the time elapsed.

2. AT-WILL EMPLOYMENT DISCLOSURE
You acknowledge and agree that the submission of this application, the scheduling of an interview, or the processing of your data does not create an offer of employment, an employment contract, or a guarantee of future employment for any specific duration. You understand that any future employment relationship with the Company in the State of Washington will be “at-will.” This means that either you or the Company may terminate the employment relationship at any time, with or without cause, and with or without prior notice. No manager, supervisor, or representative of the Company, other than an authorized officer via a formal written agreement signed by both parties, has the authority to enter into any agreement to the contrary.

3. WASHINGTON FAIR CHANCE ACT COMPLIANCE (RCW 49.94)
In strict accordance with the Washington Fair Chance Act, the Company does not request, require, or evaluate an applicant’s criminal history during the initial screening phase of the application process. You are instructed not to provide criminal history information on this application form. You acknowledge that if you are determined to be otherwise qualified for the position, the Company may subsequently conduct a lawful background check as part of a conditional offer of employment.

4. EQUAL PAY AND OPPORTUNITIES ACT COMPLIANCE (RCW 49.58)
The Company is committed to pay equity. In compliance with the Washington Equal Pay and Opportunities Act, the Company does not request, require, or seek your wage, salary, or benefit history from you or your current/former employers. Please do not volunteer your historical compensation figures during the application or initial interview process.

5. BACKGROUND AND REFERENCE CHECKS
Subject to applicable federal, state, and local laws—including the federal Fair Credit Reporting Act (FCRA) and Washington State laws—you authorize the Company and its designated agents to investigate your background to evaluate your suitability for employment. This may include, but is not limited to, verifying your employment history, educational degrees, professional certifications, and contacting the references you provide. You release the Company, its agents, and all individuals or entities providing reference information from any and all liability or damages arising out of the collection or verification of such information.

6. EQUAL OPPORTUNITY EMPLOYMENT & WLAD COMPLIANCE (RCW 49.60)
The Company is an Equal Opportunity Employer. In accordance with the Washington Law Against Discrimination (WLAD) and federal regulations, the Company prohibits discrimination or harassment based on race, color, creed, religion, national origin, citizenship, sex, age, marital status, sexual orientation, gender identity or expression, honorably discharged veteran or military status, the presence of any sensory, mental, or physical disability, or the use of a trained dog guide or service animal by a person with a disability. If you require a reasonable accommodation to participate in the application or interview process, please contact [Insert HR Email/Contact Info].

7. DATA PRIVACY AND MY HEALTH MY DATA ACT (MHDA) DISCLAIMER
The Company collects and processes the personal data you provide solely for recruitment, applicant evaluation, and hiring operations. This data will be retained in accordance with corporate data management policies and applicable legal retention windows. In compliance with Washington’s My Health My Data Act, the Company does not collect, track, or process consumer health data via this standard employment intake form. Please do not submit or upload medical or health-related records with your application materials.

8. GOVERNING LAW AND SEVERABILITY
These Terms shall be governed by, construed, and enforced in accordance with the laws of the State of Washington, without regard to its conflict of law principles. If any provision or portion of these Terms is determined by a court of competent jurisdiction to be invalid, illegal, or unenforceable, the remaining provisions of these Terms shall remain in full force and effect.
